AGREEMENT AND SIGNATURE

I hereby sign over the above named animal to D.R.E.A.M. Animal Rescue on 7/22/2026. I understand that I have signed over any and all rights to D.R.E.A.M. Animal Rescue regarding this animal. Once I return this animal to D.R.E.A.M. Animal Rescue –he/she will be under D.R.E.A.M.’s care and any decision on where to place him/her will be up to the members of D.R.E.A.M. D.R.E.A.M.’s adoption process includes applications, home checks, contracts and an adoption fee that will go directly back to D.R.E.A.M. Animal Rescue to help continue to save other animals. By signing this release form, I am agreeing that D.R.E.A.M. Animal Rescue takes over legal ownership of this animal. The animal does not have to be returned to me under any circumstances and no further communication need be provided by D.R.E.A.M. Animal Rescue.
